Need to configure WOOCOMMERCE

I need to someone to configure woocommerce with the checkout pages of our wordpress website. -There will be no merchant account integration -My client only wants the customers to see checkout page with total balance and click "Send" -The card will not process, information will be sent to admin via email The checkout page should be 1 page only and contain the following information: 1. First Name 2. Last Name 3. Street Address 4. City 5. State 6. Zip Code 7. Choose a meal plan: (Drop down box options): OPTION 1 ( $109.99 or $79.99), OPTION 2 ($119.99 or $89.99), OPTION 3 ($104.99), OPTION 4 ($99.99) 8. Would you like cookies or cold pressed juice? OPTION 1 ($6.95), OPTION 2 ($10) 9. Do you have a guard gate in front of your community? (Drop down box options): OPTION 1 (YES), OPTION 2 (NO) 10. (Check box) By clicking the check box to the left, you agree to have your card charged for the total amount below. *If you want to discontinue the meal plan service, you must cancel by Thursday at 3pm of every week.
