Build a website form -- 2

Hi, I have some data in Database/Table 1. I want to review them, add some more stuff and then save in Database/Table 3. So I need a web developer to build a web form for me. The form will ingest data from Database/Table 1 (6 columns), store it temporarily in Database/Table 2, present a form (where I can review the entries), and then save the output of the form in 44 columns in Database/Table 3. The form will retrieve data based on the date it was inserted in Database/Table 1. You should create a date field on top where I can select the cutoff date for importing records. For 9 fields, I will provide you with data in excel to create drop down lists. The form will load up to 50 rows and then offer a submit button. Once a row has been loaded into the form and a Yes/No decision made to save to DB 3, then add a flag to DB 2 so that it wont be loaded again in the form (make it optional to load them or not). I will give you SFTP access so that you upload and set it all up in my hosting package.
