Webform with some simple functions

We are looking to build a web registration form, the registration form will save all the data to a database and send also send the data to an email. This is a form for a marketing campaign where we ask people to send gifts to friends. So, there is a “Sender” which is the one that will fill out the form and a “Receiver” which will receive a post package. Basics: -The form will need to be built on Bootstrap for responsive benefits and styling, [login to view URL] -The form will be iFramed into a WordPress page. -The form will need to be easily translated into Icelandic, so all text strings will need to be in a file that can easily be accessed and changed. -The form will be set up on our server and the freelancer will need to help us get it up and running on our server. Form: -The following fields will be used in the form: --Sender ---Name of sender (text field, required field) ---SSN of sender (number field, required field) ---Email of sender (email field, required field) ---Phone of sender (phone field) --Receiver ---Name of Receiver (text field, required field) ---Home address of Receiver (text field, required field) ---Home zip code of Receiver (number field, required field) ---Message to Receiver (text area field, max charts is 200, must show how many charts has been used, demo: “Charts: 78/200”, required field) --Submit form -Form functions: --The form must have error notifications for missing fields etc. --The form will display message after the form has successfully been submitted. --The form will send automatic email to those who submit the form, the email must be sent in a HTML email. The email is sent to “Email of sender” and the admin must be able to edit it in a single html file. The email will include the “Name of sender” and “Name of Receiver”. --The form will have a limited amount of submissions, so I can choose to have only 1000 people to take part in the campaign, the number 1000 must be controlled by me and changeable. --We must show a counter at the top of the form, so it will show the number of submissions made vs submissions left (113/1000). --In the database where all the submissions will be saved I need to be able to delete a submission, then that submission should be added to the list of submissions left. --We need to show an error if same “SSN of sender” tries to fill in the form multiple time, only one submission per “SSN of sender”. --We need to be able to have the database view locked with password, so only the admin can access. --We need to be able to export the database to an excel format, (.xlsx) is preferred but (.csv) or (.xls) will be ok to.
