Software must be PHP with database MySQL. Hopefully you have done
something similar you can use as a starting base.

I will probably be integrating with PHP-FUSION or what ever other site I choose,
but that shouldn't effect this project. So you are not designing the whole website,
just the functions described here.
I want to create a directory of churches, synagogues, or orther various
types of organizations. A leader from any organization should be able
to sign-up as a new user. System should email verification code
before complete survey can be filled out. Leader then fills out
contact info and survey questions that describe the organization.
These columns always present:
1) Organization Name
8) Comments (up to 500 characters)
11) Metroplex area (for example Dallas/Ft. Worth, even if your city above is Arlington).
12) Contact First Name
13) Contact Last Name
There should also be a userid, password, email with typical register new user,
"lost my password" functions.
Additional columns should be database driven, for example,
a table called UserDefColumns might have the following values

There must be admin screens to allow for data entry (add/upd/delete) of all the above tables.
Another function is that I should be able to hire someone to enter the basic info.
There should be an email letter (stored in database) that can be sent
organizations stating that there basic info has been added, and they are requested
to complete the more detailed survey. There should be a flag in database indicating
that data has been entered internally, and a bulk email function to invite organizations
to participate in survey, along with unsubscribe option.
I have one or two specific sites in mind for this, but
would like to have the software generic enough to resell
(This is a work for hire, I maintain all rights to resell).

6) Side by side view - user should be able to select either two or three
organizations (probably a checkbox from lists 1-3 above)
and see all the survey results side by side, i.e. each
organization goes in a different column (report would be either two
