PHP: Bug fix for creating PDF (by fpdf)

I had a freelancer, who created some pdf forms by fpdf. Now we had a short update, he made some changes and since this time, nothing happens any more.
I tried to connect freelancer since one week, but without success.
Old php is still working, but without neccessary changes.
Now I need changes very urgent. You should be able to do so within next few hours.
You will get both php files and should have a look, where issues are, and fix them.
