Simple HTML create page

We need a php script that will be placed on a webserver and create an HTML file on same server (in sub directory), the new file url when posted on whatsapp, will have a preview image to a link.
very Simple clean design is needed for the user param entry page (we just supply logo and color scheme)
The script will get from user the following fields and create the new HTML file (see format below)
then it will upload the new HTML file and image to a sub directory on the web server its located on
and will display the new HTML link (for copy/paste)
Params:
XXXXX – page title
YYYYY - Description
SSSSSS – Site text url
MMMM – Mini title
Image1 – Image to be displayed (upload image)
UUUUU – Url to jump to
