user data display from user input form

Option 1st

In wordpress want to get user input by selection form, and display output from different state/province which was already selected, but dont want to store data any of them, its only shows display of all input

example:

[login to view URL]

or attached file "new Compr.....pdf"

but after this it shows display need to show other things as well like in this pic "[login to view URL]"

or

2nd option

1st create same as "[login to view URL]", and for AINP and other states.... can be created another similar form, which most of state PNP Questions are same or close to each other. below link shows all points upfront which we have to calculate ourself manually but I want user to select from the form and will display point same as "[login to view URL]" link

This is a low budget part of project, so please don't bid too high not more then $100 CAD, won't able to talk on that if its too high price. need to have it done in 2-3 days, and no further support required unless if there will be any error.

WordPress, domain, will be provided if want to work live online.
