We want a small poll app that has the following requirements:
Each user coming in will have a specific code. We assume this would be provided to the app via a CSV that it can read (list of names and codes), but this is quite flexible as long as it can be set up externally to the app.
Preferably the user would come in to the poll via a URL with the code in it so it can be sent to the user as a link.
There is only one question to the poll: "What is your favorite song?"
The question list will be pre-populated to a limited selection (approximately 12), preferably with radio selections or a similar interface. Selecting a song and pressing submit will simply say a short thank you message and not allow the user back into the survey with that code.
As a song is selected, it shouldn't be presented to future visitors: each song can only be chosen once.
As the mouse hovers over a given song, it should auto-play a clip from the song (we'll create the clips - any audio can be used for testing). How the song clip is tied to the choice is up to you - we're happy to change the clip names to match the ID of song choices or other things that make them easy to find. Also, file formats for these are flexible.
There should be a report (different URL) that shows the given name (tied to the code) and the song selection. No security beyond a different URL is needed.
It doesn't need a direct UI option, but there needs to be a way to clear the answers to reset things while testing and before releasing this to the recipients.
This will be run from a Linux box, but otherwise the requirements are flexible - language choice, etc.
