I would like to hire a Software Developer

Budget $750-1500 CAD

I need to develop an automated form for client encoutners so I can quickly document the encounter. I have a client record system that uses the following: front end is based in the web browser so the languages that I'll use are HTML (for displaying all the text and check boxes, type boxes, etc.), CSS (for positioning all of the elements on the page), and Javascript (used for any logic such as switching between different options).

The project has already started and needs to be edited and finished.
