Check Spanish Translation of Intake Form

Job Description:
We operate "Never Quit Looking", a global database of missing persons, unsolved homicides, and unidentified remains.
One of the ways we develop case files is through an intake form offered via JotForm ([login to view URL]). The platform allows us to offer an English and Spanish translation. The form gathers the victims details (age, hair color, distinguishing features) and details about their case (setting, situation, location, etc) as well as some related information (police contact details, vehicle, suspect).
We have machine-translated the form into Spanish, but we need someone who can check the translation and tell us what needs to be corrected.
The first page of the form is in paragraph format and is likely to have the most issues.
The rest of the form should be pretty straightforward for translation (hair colors, eye colors, locations, country names) and is largely single words (e.g., Nightclub) or phrases (e.g., Orthodontic Braces). Hopefully the machine translation worked pretty well on these, but they all need to be double checked.
You can actually use the link to see the form in its entirety. Just use the language change icon to switch between English and Spanish.
We hope it wouldn't to large of an undertaking. If possible, just a word document or email explaining the changes that need to be made would suffice.
We have to pay out of our own pocket, so cheaper is better though we want it done correctly.
