Write a simple logging/reporting/alarm-setting iPhone/Android/iPad app for very small mom business

I am creating a simple app with 4 functions/pages/screens: (1) "Log Screen" logging time intervals, (2) "Reports Screen" reporting those intervals, and (3) "Set Alarm Screen" setting a push notification alarm of a user-set interval. (4) "Further Resources" will allow them to access my website and/or other links.
(1) Log Screen - I need the user to be able to time from either event 1 or event 2 as a START point, THEN mark one of 3 actions by the simple push of a button (3 button choices to mark that event on the timer), AND option to add a short NOTE to that button push, the timer continues in the background until overall STOP button is pushed, as user could THEN repeat this logging of the 3 actions as many times as they want, THEN once STOP is pushed, user can generate output.
(2) Reports Screen - Output would be a log of the timed period - the start point, the intervals and which 3 actions happened - this could be printable but definitely must be save-able by date, including any notes and specific actions taken.
(3) Alarm Screen - After logging and viewing the report, the user can THEN switch to the "alarm" screen where they can set an interval alarm to send a push notification to their phone every ____ minutes that says a message that I'd customize to urge them to take the action.
DESIGN
The design of the app will be beautiful, flat design, very minimal and user-friendly, very easy to use for moms with small babies.
FORMATS
iPhone, Android, and iPad versions are a must - so you must be able to design in all 3 for this project.
