Create an html form with a bit of jQuery

This form will be integrated on an e-commerce catalogue for the client to be able to send quickly a quote request (because the e-commerce has no prices visible).
It will be integrated on the right side of product pages like this one :

Step 1
-User clicks on "Demande de prix" button. This step slides left and the first part of form appears from the right
Step 2
-User fills input "Quantité" (numerical only)
-User fills input "Email" (data validation)
-User choose radio "Livraison" ("delivery"). ==> "'oui" or "non"
Step 3
-User fills "Name" "Surname" "Phone" (mandatory).
If the user chose "OUI" to "Livraison" in STEP 2, then the fields "Adresse" "Code postal" and "Ville" become mandatory as well, because for us to be able to give them the price of the delivery, we need to know where they live.
-User fills the previous fields if required or if willing to.
Step 4
2 emails sent through "[login to view URL]":
- one to User with a "Thank you for your quote..... regarding product... "
- one to contact[at][login to view URL] with a "Someone asked for a quote + all infos about the quote".
[Thank you message] or [There was a problem message] depending on scenario.
Functionality :
- No refresh. Each "step" is slided from right to left after click (see "[login to view URL]").
- Error handling : missing info or bad info (see "[login to view URL]")
- Error handling : if 1 or 2 emails couldn't be sent
- Create the "steps" system with jQuery and CSS (play with the margins). Light code and no plugin.
- For the emailing, rely on [login to view URL]
- the form has to fit 100% width inside :
#center_column > div > [login to view URL] > div.pb-right-column.col-xs-12.col-sm-4.col-md-3
- keep code clean and clear
- Both emails have to include a link to the product page (to know which product is concerned of course)
