Contact Form and Google Maps

I need to get the contact form on my contact page to work. It uses a PHP script:

On that same page I need to get the Google map to show the correct location..[login to view URL]
I also have form or comment areas on pages:

Those don't seem to work either.
The developer is very slow to answer so I am looking for outside help with this.
