Small Visual Basic Image Voting App

I would like a small Visual Basic program that resides on the root of a usb flash drive.
The flash drive will have I have twelve folders - 1 folder for each month, The monthly folders contain pictures taken during that month
I expect to have around 4000 images total
The folder structure is
Jan
Feb
Mar
Apr
May
Jun
etc
When the user starts the program
A Userform comes up and allows the user to select a folder from a drop downbox
Once selected the User is presented with a larger form that has the first image in the folder
The bottom of the form has 2 buttons for voting - Button 1 = YES Button 2 = NO
As the user votes the next image is presented until all images in the folder are reviewed.
The users vote is recorded to a text file on the flash drive with 2 fields
Filename,and the vote = (1 = Yes) (0 = No)
Example

Note:
Each image can only be voted on once.
Program should be able to remember what folders and images have been reviewed.
Example
User may start reviewing April images at 8:00 am stop noon. The program should know where he/she left off. when he/she restarts at 2pm.
Once he's reviewed all of April
The month of April is no longer selectable in the dropdown
