Access VBA script to export Form to pdf for multiple records

Hello,
I have an Access database with an existing form. I need someone to develop a script that exports the form as individual pdf files for each record in the database. The name of the pdf file will be derived from a field in one of the tables.

I have added an example database. The script would need to export the form "Main" as a separate PDF file for each record in the "Main" table. The file name of the PDF must be the value in the "Contaminated Site ID" field of the "Main" table.
The form and tables in the example database are only an example.
It needs to work in Access 2007.
