Webpage with graphics design and programming

I need a simple web page constructed, with professional looking graphics, to resemble the pages [url removed, login to view] or www.green-tea-purity.com. These pages are not mine but are good examples of the sort of look and feel I am after.
A clean, professional, look is essential.
I will provide the copy, and some of the images, along with a basic layout of what I require set out in Microsoft Word or PowerPoint. You will need to take this document as a guide to actually designing the page, finding or designing any other required images and associated graphics, so basically your job is graphic design not copywriting.
However, a copywriting background or experience is desirable.
I will also cover the cost of any images purchase from istock or other, but please seek my approval for these purchases first.
I will also provide the form code for the name and email opt-in boxes.
You should provide me the complete html page with images in a separate subfolder. Original graphics files for any graphics you have constructed should also be provided to me at the end of the job, preferably in psd format.
I need the job completed in 1 week maximum, including early site of drafts to allow me to comment and ask for revisions.
Overall, I'm looking for a responsive, first class web designer with sales or copywriting experience desirable, who I can develop a long term relationship with for the future!
