Web based animal management utility layout design

I am looking for creative webdesigner which will develop a layout for animal management utility for me.
What is currently needed is an basic layout:
- General webpage Look and Feel in a form of a picture (to be provided before anything other).
- Home page / Static page
- Animal CRUD (list with pagination, detail with in-place editing)
- HTML Mail Layout with same Look & Feel
- 2 animal pedigrees in a form of HTML based A4 printout (simple black and white and one with colors)
Layout must be HTML5 valid and be built upon Bootstrap.
