Formidable Froms - Custom Views for score survey

Hello,
We have a WordPress website that uses Formidable forms. We have a form that is a survey where people select ten questions where options are 1-5. In the end, after form submission on the result page, we have five different result views:
14-19
20-29
30-39
40-59
And for each result, we want to display a different graphic on the result page. See attachment.
Right now, the results are coming through an HTML table, but in Mobile, that looks terrible. We want have a graphic image instead.
If you are super familiar with the Formidable form and feel you can do this successfully in the next 24 hours. Please say what 1+1 is at the beginning of your proposal.
