So on the link above, I need to create a similar interface in WordPress, where the customers will have to choose the checkboxes and radio buttons. Once the customers complete the order, it will need to be formatted into a report that will be emailed to the owner of this business so that they can prepare the order. We already have a database of items, which needed to be filled out by our customers.
