Word Press Widget

Build "Modify" the widget on [url removed, login to view] Yellow Page Search. This widget works great on this site but will not work on other themes. I need it so I can add it to other themes as a true Word Press Widget . See coding on View source of the page.

As you can see it was built for one theme only.
