Need help to fix my home health website

I already have a website designed, but previous designer didn't include a backup file so I can update or fix anything with my frontpage software. Every contents are already in there so you might jsut have to cut and paste. I do like the design I already have, but I still need some changes in term of the logo or couple of the pictures. Lastly, I would need forms input to work so that people can do referral online or submiting requesting information online. I will certainly describe of the type of logo if you have any suggestion. I do have couple different logos for my site, but I dont think they are very professional or well suite.
It is very important that you should create a template so that I can add any additional pages in the future or I can fix it if I discover any data entry error.

I also need that you are good with English because I would need you to edit/proofreading our contents and possibly brainstorming our company profile content. Your suggestions on "keyword" would be excellent.
I also need post support if I have problem or questions while editing/updating my site. I dont think I would have any problem after the first week, since I am some what computer literate. This would be a plus. However, my website would be really simple and wont take less than 1 day. The project completion date is very flexible.
I will be ok to have your company or notation at the end of the page i.e "this page was designed by ......), or it can be in the "view sources"
