Make a fillable form in pdf

I attached the pdf file.
- Convert the form to be fillable and can perform a simple calculation (Line Total, Subtotal and Total). You can re-create the form but the layout has to be close to the attached file.
- Translate it to Portuguese (You may use Google Translate)
- Insert the form to the first page of another pdf file which I will provide
- If possible, when I enter the Prod ID, the description and price will be shown automatically.
This is a simple job. I will pay max. $10 for this job.
Please convert some part of the form into fillable, translate some part into Portuguese and make the form can perform the calculation as required. Attach the form in PM.
