Update Custom VBA script to incorporate new CSV input format and features

We have an existing VBA script for Microsoft Excel that processes a CSV input and produces an Excel file as output. However, the format of the CSV has changed slightly, as have our needs, and we'd like the script to account for those changes. (The CSV file is produced by Squareup.com.)
Changes:
* For input files not containing the "Coupon" variant, we'd like the output file to exclude the "Coupon" columns
* The input format of check transactions has changed slightly, and as such, they're currently being mis-categorized by the script. (They're being included in the "other card" column, rather than the "cash and chq" column)
Wish-list Changes:
* Accurately account for refunds based on payment ID
* Ignore "0.00" transactions in summary
In the attached files, [login to view URL] is the current script. Other files are sample input and output (both current output and ideal output) for a variety of scenarios.
