Need Tutorial On How To Create A Custom PDF Form

I am looking to create custom real estate pdf forms for my clients.
The user should be able to change text and images while using Adobe Reader.
I would like to be able to choose any font I want when creating these forms.
I want to create something like this: [url removed, login to view]
After the tutorial is given to me I would like to be able to ask a few follow up questions to make sure I got it :)
I currently have copies of the latest Adobe Acrobat and InDesign.
