Simple Email Form
I need a simple email form installed and modified to match the font / layout of my existing website.
Work Required:
1) Create a simple email form:
a) Collect the following from user: Name, Company, Email Address
b) On User submission email user a predefined email with a specified PDF attachment
2) Modify form to match fonts and design used on website
3) On user submission, email attached PDF document to submitted email address
