Create MySQL Database and php pages for playlist

I am creating a site for a radio station and need to have two databases for the following activities:
(1) A playlist feature
(2) Record Charts
I have a basic outline for the two databases but need in the first instance the page created for playlist feature. The database is relation with the header information such as DJ, date, show title etc in one table and then record, artist, record format etc in antother table.
As track are added we want each track stored also in another table so that AJAK can be used to display tracks as people type that match to speed up input for tracks already keyed in.
PLEASE NOTE
This project is not to create a whole site, just a couple of pages to work with database tables I will supply. Also I only need the input and edit pages created with just simple layout.
