It's a script modification job.
Only 3 hours of the work.
you must start now, and complete that within 3 hours.
There are 2 forms built, need to combine those into one application! and give them the same layout. using Ajax submit instead of the current PHP redirect.
one of those forms need to be separated into sections ( Accordion, and adding jquery datepicker for the date field ), and some other issues like that. Other simple tweaks will be added!
Price / timeline is NOT negotiable!! No Milestones, until the job completed!
