Project scope: 1. Convert a Word document to HTML. This will have to be done by hand because I do not want all the extra code that Word inserts when you simply click File -> Save As -> HTML. And there are numbers along the left hand side of the document that do not export when that method is used. These numbers MUST be part of the new HTML file. 2. Insert text boxes/textarea/radio buttons, etc in the appropriate places for data input by the user (HTML version). These inputs should take the place of the current inputs so no lines should show beneath the text boxes. Include name, size and maxlength attribs as well. Please use lower case for all element names and make the names mean something (not just field1, field2, etc). 4. Create a MySQL database table for the contract with the appropriate field names to tie back to the HTML doc. Please use all lower case for field names. 5. Add a submit button to each HTML file and the appropriate PHP code to write the data to the database table. After insert, spool the page back with the values just submitted in their original text boxes. There are no default values so if nothing was submitted for a particular input then nothing is placed in the input when the page spools back.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
