HTML page with some java

I would to have someone take a .gif file I've created, turn it into a php-generated xhtml page, with some java included for a handful of dropdown boxes within the page. Additionally, the text on the html page is all pulled from mySQL tables, and will have to be written into the code. Finally, a lot of the entries dynamically hyperlink to other pages, and those will need to be included as well. All code should be well commented upon.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

I have included the .gif file that I want duplicated in a php-generated page using xHTML strict 1.0. The menu along the top that contains "summary", "financial results" and "product/divisional results" has already been created and is a separate file, so you are responsible for the file beginning with the text "headwaters inc." The trickiest part here are the dropdown boxes. Everywhere there is a "+" in the .gif file, that indicates that there is more information to be displayed from the database table that can't fit within the allotted space. Clicking upon the "+" should drop down a box, with the same width and color as the current column, and long enough to fit the remaining entries, along with a "-" similar in style to the "+" that will allow the user to contract the box. Ignore the "X" that you see in the file, it should be written into the code that only if there is an overflow of information will a "+" show up, otherwise, it will be blank. This piece should be written in javascript so that it can function without having to reload the page. Beyond that, everything is pretty straightforward, and I will provide you the table names and column names for all of the text that is pulled from the database, along with the page destinations for all of the dynamic hyperlinks.
