PHP & MySQL - Update Record From Checkbox

Dear Coders,
I am in need of a simple edit to my page. The page will list information from my MySQL Database, on each record at the end there will be a CheckBox. What i need to happen is if the Approve button is selected then it will update all the records that where checked and update the status of the records in the Database to Approved. If they select the Decline button then it will do the same but will update the status to Declined.
I've attached a picture for you so you can view how its laid out. This work must be completed within the next 6hrs. Any questions just ask. It is a simple/easy thing to do but i dont have the time to finish this off.
