simple order form for Virtuemart ( front end )

A simple order form is needed to be used by customers who dont want to register and dont want to go through virtuemart check out process.
Except basic information like name, address, phone num, email etc, the form needs to have ~5x drop down lists for the customer to select product SKU and quantity. When SKU is selected the form has to show the product name, price, and attributes if any, for each selected product. ( the product attribute has to be selectable also )
If possible it will be nice for the form to display the total weight of the products and calculate shipping charges. Calculation will be simple based on a flat rate per Kg for all destinations
2 extra fields are also required, a simple antispam captcha box and an "agree with terms" checkbox.
After submitting, the form has to email the user and the admin with all order information and most important to add an order entry to virtuemart backend with all the order details.
Site is live and in Greek language.
The form needs to perform basic field validation.
If possible we would like to use RSjoomla RSform! Pro (which is already purchased) and provides all the basic form functionality and permits scripting for programming the extra features.
