We need a contact form that puts results in a database. This should be a php program that send data to mysql.
The form will have simple fields such as
name
country
phone
comments
Plus a check-box field:
type1
type2
type3
After the data is written to the mysql it should return a page "thank you".
This project includes the creation of the MySQL database.
This project also includes the creation of a simple PHP admin screen that is password protected. This panel must include a button to export the database to Excel, by date range. This panel will not do anything else.
The PHP script and MySQL database will be created on your server and we will upload this to our server. No FTP, MySQL, or PHPMyAdmin access is available for this project. You will provide the mysql scripts, the php form script and any other items so we can install.
