PHP Script for Uploading Files and emailing results.

I need a PHP script which does the following: 1. Prompts user to upload a file of .mp3 or .zip, with no limit. It must be placed in a directory specified in the script file. 2. Next button must be clicked to proceed to next step. 3. A form will be presented to ask for: Full name, valid Email Address, Website URL, Type of Technology (edit box), Description message box, and checkbox for accepting terms of service. 4. After clicking Preview, the user can review the results or click back to edit. Else: User can click Submit. 5. A confirmation page will show that the message was sent. 6. The admin will receive the email with the prior submission info, as well as a URL to the uploaded file and IP of the individual. Notes: If upload fails, user will not be taken to submission form. Please convert line breaks in emails.
