Need to develop a program for basic electronic medical chart

I dont know much about software program type. However, what I need is a basic on-site software program for our nurses to chart and record the informations. I will need your consultation to see how we can have it in a secured web-based if possible. If not then I can have them as onsite in a secured server (need suggestion on which server to use).
The program would have:
tabs on the top where user can just select.
I have all of the forms in word and pdf that would need you to have them live or clickable and it will save and print in form. I would need them basic enough that if I need to creat additional client files I can just add the template.
It would have a list of clients on the right panel for user to select. When they click it would populate/simulate like a chart/file folder. I would work with you on naming them. Everything will be save and they would appear just like a book/file folder that user can go back and read.
Scanning upload capability so I can upload all of our current files.
Also need to have a sign in/out procedure with date stamped and electronically signed.
Real-time assignment populated (reminder) what have not been done integration.
Program them in a way that I dont have to reenter the information again. (name, address, md info, diagnosis etc...)
Create a program for punch in/out time card that would import to excel or work with quickbook. Excel would prefer.
Employee file that would be just like the clients' file that only admin can access. Of course I will work with you on what need to be included.
All files must be able to save in word/pfd/excel format and I can print them out if needed.
Budget is set for $5000 and I need them done in 2 month. Possible contract for tech support/maintenance on a monthly basis to ensure they are working.
