We are in need of a editable online or offline form that can be created with data entry points that once completed convert the information into visual product. Much like database forms can create pie charts and bar graphs.
What is this for?
This form is for Inspections with many different data entry points accessed online via our website.. Most of these entries are radials or check boxes; however there will be text boxes as well.
Online Entry Form:
1 Section with 5 different radial/check box entry points, 2 sections each with 5 subsections with 5 different radial/check box entry points, and 6 text boxes for name, inspector number, VET report number, vehicle information, vin number.
1st Section Would display as "Overall Outlook" would show a graphic depending on which of the 5 radials on the form options where selected it would trigger the bar graphic, which is divided into 5 equal part, would amount to one or more part of the bar being selected at a time displaying in a gradient bar graphic.
2nd Sections would repeat the first but have a title "Live Testing" and the second "Static Testing" with below indicated subsection title followed by a gradient graphic bar as a result of the selections as in Section 1.
These two sections data entry form would produce the above results in an jpg format for downloading and printing.
The text boxes inputs will not be displayed on the produced graphic but be sent together with a copy of the resulting jpg graphic to a specified email for storage on our serves for product verification.
In addition I will need the seal graphic that you will see on the attachment to be redone. Something similar to what is in the rough edit but just cleaner and more official. You will have some freedom but with that as the starting and foundation.
I apologize that I am unable to describe my needs further but I am willing to provide diagram for visual aid and discussion of project in detail with interested freelancers.
