PHP/MySQL application with Bootstrap front-end

Project Description:
Currently I use a PHP/MySQL application to create some 6 different forms and collect data (appnitro machform). I am looking for someone who can develop a web based application that will be used to process the submitted data.

In the new application an admin should be able to create a few accounts and assign forms to user(s) that can process it's data. These accounts should list and review the submitted forms (display the questions and submitted answers) and add 5 fields (text box and a few checkboxes) with comments to each form. The form should then be e-mailed (via configured smtp account) as a PDF-attachement to a prior per form specified address. In the PDF the username of the reviewer also has to be visible.

I'm looking for someone who can do this well, secure (submitted data can be sensitive - I want this to be safe), and can make it look good. Not long after I would like to expand the application to replace the machform component and have the new application to completely service our needs.
