Need video upload script

We need a simple video upload script written in PHP that performs the following.
- Allows the user to browse their computer for a video file and select the file.
- Only allows uploading of .avi, mpeg, mp3 formats. This should be commented and changeable in the code so we can add/remove file types if needed.
- Should limit up to 10 meg file size. This should also be commented in the code so we can change this dynamically if needed.
- Should save the uploaded video to the web server and return back the saved location and file name (url).
Large uploads should be able to process without locking up or hanging the user. A progress bar or similar should be presented to the user as the upload is happening. Large uploads should also be able to be done without hanging up the server. The script will run on a dedicated server but it needs to be written so it can handle large 10 megs uploaded videos well.
You don't need to add any styling or design to the script because we will be adding it to an existing page via an iframe.
