I need both of the forms built (see attached). Please note that I DO NOT need the whole web page built. The page templates are done - I just need someone to build the forms in that right hand column. The attached images are just mockups of what I need in the form. You will use the font style already defined in the pages. Please don't make the spacing like it is in the mockups - just lay it out normal :)
The contact form is pretty straight forward, but the other one needs some sort of a javascript calendar and three browse fields. I will worry about where the information is sent and how to store it in our database. I'm just running behind and don't have time to get all of it done.
I need someone who is available right now and uses a chat client (gchat, msn, yahoo, or sykpe). Thanks!
