Spa Ipad App with form for registration

We are opening a MED SPA and I want to have the form into the Ipad.

The idea :

The client comes, fill up the form using the ipad, when press the button Submit, the information is saved inside the ipad (where only us have the access of the info through a password), we can export the database as CSV or even better if the app can communicate with a mac software so we can edit the information in the computer if we need to (and for backup reasons too)

The Form:

This is the most important thing- I want it in a way I can customize the form myself ... Adding the fields and choosing if its radio, checkbox, e-mail, calendar, only numbers ...

If thats impossible, I could send all the forms to you, but I would like to have a way of adding fields as we grow as we need more information for different services.

I want a comment field that we are the only ones that can see and edit infos as the client comes, like she paid 300 dollars and she still has 200 in credit or give her a discount of N% for bringing this person or this person has this type of skin so make sure we do this ... Inside infos ...

A PDF or a text (insurance or contract) the person reads, we can add the name of client, witness, date and can SIGN at the bottom;

E-mail option - Email all the clients at once with a picture of a promotion or only one client to let them know about their booking, appointment or a special and private message

Extras (Optional)

Booking with time of service (if the service takes 10min, 30 min or 1 hour...) and area showing the available spots and upcoming clients;

Make a payment using one of those credit card device for ipad;
