PHP/MySQL - dynamic pages from MySQL db

The DB is: country, city, code
I need to be able to extract the data from the db using PHP.
Ideally, the PHP code should generate a new php page from each record of the database.
For example: Ireland, Dublin, 353 - into a new page from a template: [login to view URL]
I use Dreamweaver and i would like to have a php code that would do what i've described above.
Open to any other suggestion that provides me with the same results.
