Simple jobs add video, order button, optin form and popup to site

This is a very simple job.

I'm looking for someone that can do the following within the next 2-3 hours

#1- Add our name and email address form,

Similar to how the name and email address form is added to this page:

(we'll give you the html code for the form)

#2- We need you to add a pop up code that comes up if someone trys to click onto the video image that

you see on [url removed, login to view] BEFORE entering their name and email address. (just as it happens over at

[url removed, login to view])

And on this page [url removed, login to view] :

#3- we need you to add our video: [url removed, login to view]

(just as you see it added on [url removed, login to view]

The video should be placed just as you see it on this page:

[url removed, login to view]

we'll give you a link to our video on Amazon S3 in either a flv or mp4 format

#4- I need you to have our order button popup under the video after 2700 seconds just as it does on this page [url removed, login to view]

(wait for 1 minute and you'll see it pop up under the video)

All very basic and simple stuff, that I would normally do just dont't have the time so I am looking to have you do it.

Please bid fairly and only if you can do this NOW in less than 2-3 hours
