Keep Input Checkbox But Incorporate it into a Dropdown

I have a PHP file that loops the input checkboxes:

I need to keep the checkboxes but instead of listing them in a column, I want them in a dropdown. I can provide FTP access to the file and URL for testing. It is on a Wordpress site.
