wordpress mutistep form plugin

we want to use wordpress boilerplate for the development. if you dont have experience in this.. do not apply.
the plugin will show a multi step form in the frontend for the user to input details. each step may contain radio button, checkbox, input, textarea, select combo. at the bottom of each step there will be previous and next buttins to take to each step. at the end there will be a save button.
there are four pages: personal details page ( firstname, surname, date of birth, gender radio buttons: male / female)
address details page ( house number, street, city, zip code, country)
contact details page: email address, mobile number, whatsapp number)
other info page ( start date, start time, education checkbox [secondary school, high school, bachelors, masters], comments text area) Submit button.
when the form is submitted the fields should be validated using php and not javascript.
at each stage the form should be saved in the database.
in the first page there will be only one button <Next>.. in the last page there will be two buttons: <Previous>, <Save>
there will be two short codes for frontend: [input_user_details] and [view_input_details]
[input_user_details] will show the form page.
the view page will show in a tabular list all the details , and must have pagination.
At the end of each row of details, there will be a delete button. On clicking the delete button, the details will be deleted.
we prefer the development to be pure php script.
the form, before saving, will validate the inputs.
on activation the wp_form_ details table will be created with the above fields.. on uninstall the table will be deleted.
delete: the user may delete the details from the list. so at the end of the details there will be a delete button in each row.
