Conversion of Word Book to Indesign + cover formating

This job entails taking an 11 chapter MS Word formatted book and transferring it into Indesign (freeflighted, pdf, and ready for digital or offset printing).
(We would like to work with someone who is extremely comfortable with Indesign (CS4) and MS Word programs- 2003 and 2010 versions). Also, someone with experience with this type of book conversion (for print books) is preferred.
Book will have approximately 400 pages.
Because the margins of the actual indesign format will not be exactly the same as the Word documents from which they will be converted or transferred, some layout adjustment work will be involved to match the original layout for each chapter.
Low resolution pictures are embedded in the word chapters to show exact placement for the final indesign copy. The pict files will be supplied as separate cropped and ready files.
Cover will have to be formatted. We will supply the art work, text and desired layout.
The book will have a forward, table of contents, chapters, possibly an afterword, and index.
The final Word docs should be ready within the next 2 months.
