Simple Wordpress Quiz Plugin/Function

I need someone to create a plugin that functions as the quiz in this landing page:

The quiz should be easy to edit with a wysiwyg editor, have the transitions, timers, fake loading-delays, etc.
CSS formatting should be identical.
It should also have simple conditions like
If Option 1-A selected, then final screen should be X
If Option 2-B selected, then final screen should be Y
etc
I should be able to edit what I want in the text easily, edit the conditions easily, and edit the final screen easily in the wordpress admin dashboard (including the CSS).
It should be responsive with both Desktop, tablet, and mobile. (the main priority is mobile).
