Set Up "FileChucker" on my site

I have purchased an upload script called "FileChucker" written by [url removed, login to view]
I thought I might be able to set it up myself but I found that the options read
like the dictionary and are just as long!
I need this set up for me on my site with the options that I need...
Field for address
Field for email
field for uploaded file (of course)
The address and email MUST be saved with the uploaded file so I will know who uploaded it.

The options are MANY and we will need to collaberate on which ones I need that I am
not aware of like security.
Should be a breeze for anyone proficient at AJAX and PHP.
