Contest Brief

I need a Photoshop template, attached is a photo of a sticker that I need.
I need a PSD file with all the layers so I can add/change information.
You don't need to worry about the text, I will change the text and add logo, maybe move things around a bit and so forth after you provide me with the skeleton I need to work with.
This is for a inspection sticker, a flyer was the closed description I could find.
Size should be 10cm high, 15cm wide
