Contest Brief

We need an order form created for our new company that sells baby layette to retailers. Order form should be clear and organized and portray all our products in little swatches and pictures of each product.
Should include:
1. contact information (name of store, contact person, address, phone number)
2. payment information
3. list of items (in table or grid format) with picture of each product, model numbers, color options (shown by name of color and color swatch), size options, quantity to order 4. Total number of ordered items and total sum.

Little swatches of each item should be pictured (which I will send you separately once you agree to job). It should be organized clearly and neatly so that every item and color is clearly seen and accounted for, and so customer can easily order the different styles, colors, or sizes. Look book with different items, and model numbers are attached so you can better understand our products.
