Easy project with all of the necessary materials.

I have a resource guide that highlights 5 different websites for widows (see attached Word doc). I have the following requirements:

1. A 1-page pdf document in portrait orientation.

2. The attached logo needs to be included.

3. The hyperlinks need to work in the pdf file.

4. The resource guide needs to have an appealing look to it. (See attached Widow Health Insurance Infographic for an example of the type of style I would like).
