single page HTML from an existing site

I need to create one page HTML exactly based on [login to view URL] page
I do not have any FTP detail of site I mentioned above.
You need to design DIV based HTML page with separate header, footer, left and middle portion.
I need following.
1) One the top, i don't need the boxes "nursing profiles", "Search By State" and others.
2) On the top, instead of the "Nursing Schools". Put "CNA Classes". Also take out the symbol in between nursing schools.
3) Also, I don't want the "featured program" boxes there.
4) I will provide header image. Do not need 2 boxes in header.
5) Also, don't use the "Degree finder box" like this website. I want you to make a heading that says "Find CnA Classes", then follow with a white box that prompt to enter the "ZIP CODE" , then follow with a red "SEARCH" button.
This is the format used for in [login to view URL]
6) Clean coding, do not give me by doing "save web page as..." I could also do it rather posting this project.
